Overview

The base of the Reliability Pyramid represents the underlying problems that are the cause of near hits, events and catastrophes. If you solve these underlying causes, the consequences are reduced or eliminated.

As you move vertically up the pyramid, the categories increase in severity, from Near Hits to Minor Events, Major Events and, finally, Major Catastrophes.

You will also note that the width of each category characterizes the likelihood of that event occurring.
